The Discharge Medicine Review service provides support to patients recently discharged between care settings by ensuring that changes to patients’ medicines made in one care setting (e.g. during a hospital admission) are enacted as intended in the community helping to reduce the risk of preventable medicines-related problems and supporting adherence with newly prescribed medication.
As a patient, this service will help you with understanding the changes if any made to your drugs and improve your knowledge of the drugs that you are using. It is also an opportunity for you to ask any questions about your treatment. As experts in medicines, our pharmacists are well placed to help you achieve the maximum benefit from your medicines.
So if you have been discharged from hospital, ask in-store if this service will be appropriate for you.
